This form can be used to request the retrieval of materials from the journal store and the central store. The items requested will normally be available within one working day for collection at the library desk.

Please fill out a seperate form for each title you require.

Please note that this form is submitted by e-mail.

Once you click on the submit button you will get the following message:

This form is being submitted using e-mail...You may continue or cancel this submission.

Click OK.

You will then get a message that says:

A program is trying to send e-mail on your behalf...

Click Yes to send the e-mail.

If you do not have access to Microsoft Outlook on your PC, you will need to save the page to your PC, and send it in as an attachment, or print it off and hand it in at the library desk.
